Overview
Your first visit is your opportunity to get to know our practice, see our facility, and meet our office team. During your initial appointment, we’ll discuss your dental concerns, collect the relevant health history and background information, and perform a clinical exam to establish a baseline for care.

We’re dedicated to helping our patients maintain optimal oral health and achieve their cosmetic smile goals. We tailor each treatment plan to address individual needs and fulfill expectations of care. You can count on every member of our friendly and experienced office team to pay close attention to your comfort and address your needs with kindness, patience, and compassion.
Patient Forms
Paperwork is sent electronically upon scheduling. These can be sent via email or text.

This allows for other patients to be scheduled into that appointment time. If you miss an appointment and fail to contact our office within the required time, this is considered a missed appointment.
Our cancelation fees are as follows:
- First Missed Appointment: Excused
- Second Missed Appointment: $55 fee (cannot be billed to insurance)
- Third Missed Appointment: $100 advanced deposit towards scheduled treatment. (deposit forfeited if scheduled appointment is missed)
- Fourth Missed Appointment: Patient subject to dismissal from office
