Patient Forms

Our forms are all electronic.  New patients making an appointment will receive an email or text message automatically upon making an appointment.  If you need the link resent please call our office at 219-333-3368 or email us to contact@rifaidentalgroup.com and we will gladly resend you the link.  We appreciate your diligence in filling out your forms at least 48 hours prior to your appointment to allow enough time for insurance verification. 

Patients unable to fill out forms electronically have the option of filling out new patient forms in office.  Please arrive at least 15 minutes early if you plan to fill out forms in office.

 All forms filled out same day or in office will lengthen your wait time.
